EGFR和ALK靶向治疗:当下与将来

用于晚期或转移性肺癌的化疗发展缓慢,在1948年至21世纪初期间进展有限。在此之前,仅有很少达到了长期OS获益,因此对于化疗的益处一直存在争议。幸运的是,分子靶向药物正在彼时兴起。过去二十年间取得了惊人的进展。如今,许多药物可用于治疗癌基因驱动的肺癌患者。

EGFR TKI治疗

起点:吉非替尼(gefitinib)和厄洛替尼(erlotinib)

靶向敏感EGFR突变的药物包括厄洛
替尼、吉非替尼、阿法替尼(afatinib)、达克替尼(dacomitinib)、埃克替尼(icotinib)、奥希替尼(osimertinib)和nazartinib。2002年公开的II期数据首次证明,每日一次口服吉非替尼在化疗失败后有实质性应答[12,13]。2003年,吉非替尼获得了美国食品药品监督管理局(FDA)的加速批准,作为铂类和多西他赛化疗失败后的单药治疗。2004年,Lynch等人发现表皮生长因子受体的敏感突变是NSCLC对吉非替尼应答的基础[14]。这标志着实体瘤中精准肿瘤学的开端。
然而,几年后,直到2009年报道了IPASS试验之后,TKI治疗才写进NSCLC一线管理的《NCCN实践指南》。在这项亚洲研究中,1,217名未经治疗的患者被随机分配到吉非替尼组或卡铂加紫杉醇(paclitaxel)组[15]。试验人群包含两组,一组有EGFR突变,而另一组没有。在EGFR突变组中,吉非替尼在PFS方面显著优于化疗(HR,0.48;p < 0.001)。另一方面,没有EGFR突变的患者在接受化疗时效果明显更好(HR,2.85;p < 0.001)。
从2010年开始,四项随机化一线试验在EGFR突变选择性患者中评估吉非替尼和厄洛替尼对比化疗的疗效。OPTIMAL研究发现,与化疗相比,使用厄洛替尼的PFS风险空前降低(HR,0.16;p <0.0001)[16]。2013年,NCCN指南(版本2.2013)提出了吉非替尼和厄洛替尼作为EGFR阳性NSCLC一线治疗的1类推荐。

第二代药物阿法替尼和达克替尼

阿法替尼是首个面世的第二代EGFR TKI。在LUX-Lung 3和6试验中,阿法替尼一线治疗与细胞毒化疗进行了比较[17,18]。两项试验均显示PFS改善,阿法替尼确定为等同于厄洛替尼和吉非替尼的一线选择,作为根据NCCN指南(版本3.2014)的1类推荐。值得注意的是,根据LUX-Lung 3和6的综合分析,脑转移患者也从阿法替尼获得显著的PFS获益(HR,0.50;p = 0.03)[19]。对于存在或不存在基线脑转移的患者,阿法替尼的至CNS进展中位时间均比化疗更长。
LUX-Lung 7试验证明第二代疗效优于第一代[20]。在这项头对头研究中,与吉非替尼相比,阿法替尼显著改善PFS和缓解率()。在ARCHER 1050试验中,第二代药物达克替尼同样得到了优于吉非替尼的PFS改善(14.7与9.2个月;HR,0.59;根据独立审查p < 0.0001;),但脑转移患者被排除在本研究之外[21]。

奥希替尼:AURA3和FLAURA

对EGFR TKI治疗的初始应答通常是急剧的,但耐药性随着时间推移而出现,导致复发。在一半的病例中,继发性EGFR T790M突变是获得性耐药的基础。第三代EGFR TKI奥希替尼被设计用于靶向这种突变。在一线EGFR TKI治疗后进展的有T790M突变的患者参与了比较奥希替尼与铂类化疗的III期AURA3试验[22]。使试验允许稳定的无症状CNS转移。
主要终点为研究人员评估的PFS。奥希替尼在该试验中优于化疗,其中风险降低70%(PFS 10.1与4.4个月;HR 0.30,p < 0.001)。克服T790M突变是肺癌治疗中另一项有意义的突破。此外,根据子集分析,奥希替尼治疗脑转移患者疗效令人鼓舞[23]。对于奥希替尼和化疗,CNS总缓解率分别为70 %和31 %(p = 0.015)。7名具有CNS病灶的患者在基线时有软脑膜转移;其中四名通过奥希替尼治疗后达到了CR或PR,三名为稳定疾病。与这些令人鼓舞的数据一致,NCCN指南(版本4.2017)建议将奥希替尼用于具有T790M突变以及无症状和有症状进展的患者。
FLAURA试验在常见EGFR突变患者中对奥希替尼一线治疗进行了评估[24]。对照组接受吉非替尼或厄洛替尼治疗。方案允许稳定的CNS转移。主要终点为研究人员评估的PFS,奥希替尼治疗的PFS更优(18.9与10.2个月;HR,0.46;p < 0.001)。独立中央审查得出了相当的PFS结果(17.7与9.7个月;表)。因此,NCCN指南(版本9.2017)将奥希替尼与吉非替尼、厄洛替尼和阿法替尼一起纳入一线选择。

一线治疗选择的问题

鉴于选择的多样性,个体治疗决策可能存在困难。LUX-Lung 7、ARCHER 1050和FLAURA试验的间接比较揭示了这些研究之间的一些差异()。有争议的是,FLAURA的对照组中使用的吉非替尼和厄洛替尼不是最优的药物。在另外两项研究——JO25567和NEJ026试验中获得的数据表明,除厄洛替尼外联合贝伐珠单抗(bevacizumab)相对于厄洛替尼单药治疗显著改善了PFS(JO25567:16.4与9.8个月;HR,0.52;p = 0.0005;NEJ026:16.9与13.3个月;HR,0.605;p = 0.0157),将中位结果提高到在ARCHER 1050和FLAURA中观察到的水平[25,26]。
此外,用药顺序问题仍然存在,因为在一线治疗失败后应当存在补救手段。Ferrara等人在综述中指出第一代或第二代EGFR TKI之后使用奥希替尼而不是首先使用奥希替尼,可以达到最长的PFS(图3)[27]。除了细胞毒性化疗之外,目前在奥希替尼治疗失败后尚存在不确定性的选择。在伴随或不伴随后续化疗的情况下,TKI相比于奥希替尼的用药顺序仍有待观察。
生存率可能是回答治疗选择问题的最佳替代指标。然而,来自AURA3和FLAURA的OS数据尚不成熟,而来自其它试验的数据往往令人感到困惑。到目前为止,唯一有生存获益的头对头试验是ARCHER 1050(达克替尼和吉非替尼分别为34.1与26.8个月;HR,0.76;p = 0.0438)[28]。
JO 25567研究显示厄洛替尼加贝伐珠单抗与单独厄洛替尼之间的OS无差异。在比较吉非替尼加化疗与吉非替尼单药治疗的NEJ009试验中,联合组患者OS有改善(52.2与38.8个月;HR,0.695;p = 0.013)[29]。根据LUX-Lung 7的事后分析,约五分之一终止使用阿法替尼或吉非替尼的患者随后接受了第三代EGFR TKI,包括奥希替尼、olmutinib和rociletinib在内的治疗[30]。其中,两组的3年OS率均超过80%,且阿法替尼组尚未达到中位OS(吉非替尼组为48.3个月;
HR,0.49)。
目前,一线治疗选择仍处于讨论之中,且必须就奥希替尼的使用顺序达成一致。II期EORTC 1613试验试图通过比较使用奥希替尼直至进展与吉非替尼随后使用奥希替尼的顺序来寻找答案。

具有EML4-ALK重排的肺癌

克唑替尼(crizotinib):长期标准

第一代ALK TKI克唑替尼于2011年获得批准,多年来一直是晚期EML4-ALK重排NSCLC患者的唯一靶向一线选择。初步试验显示总缓解率为57 %,在6个月时,72 %的患者无进展[31]。III期PROFILE 1007和PROFILE 1014研究提供了进一步的证据,分别在二线和一线条件下成功地比较了克唑替尼与化疗[32,33]。

色瑞替尼(ceritinib)和艾乐替尼(alectinib)

然而,对于抗EGFR治疗而言,耐药性总会出现,这需要有效的后线治疗选择。CNS衰竭是EML4-ALK重排肺癌患者中广为人知的复发模式。
第二代ALK TKI色瑞替尼在同时招募ALK抑制剂初治和ALK抑制剂经治的患者的I期ASCEND-1试验中表现出明显的抗肿瘤活性[34]。2014年,NCCN指南(版本4.2014)推荐在克唑替尼治疗失败后使用色瑞替尼。三年后,根据在未经治疗的患者中比较色瑞替尼与化疗的ASCEND-4研究,色瑞替尼获得了针对ALK阳性NSCLC的一线治疗的1类推荐(版本6.2017)[35]。根据独立审查,试验组的PFS是化疗PFS的两倍(16.6与8.1个月;HR,0.55;p <0.00001)。
同样,使用第二代ALK TKI艾乐替尼也观察到令人印象深刻的结果。该治疗在I/II期条件下引起55 %的客观缓解率,即使在接受过大量预先治疗的患者中也显示出CNS活性[36]。基于这些数据以及确认艾乐替尼的强效疗效的II期NP28673和NP28761试验[37],2015年FDA批准授予在克唑替尼失败后使用艾乐替尼。最后,全球性III期ALEX研究确定艾乐替尼作为一线药物[38]。与克唑替尼相比,其带来令人印象深刻的PFS改善(未达到与11.1个月;HR,0.47,p < 0.001;图4),并且在具有脑部病灶的患者中同样显示尤佳的活性。在基线时具有可测量CNS转移的患者中有81 %在CNS水平上缓解(克唑替尼为50 %)。NCCN指南(版本7.2017)推荐艾乐替尼作为优选的一线选择。

治疗顺序:改变的时代

在一线艾乐替尼、克唑替尼或色瑞替尼失败后,可考虑后续使用色瑞替尼、艾乐替尼或brigatinib。Brigatinib经克唑替尼治疗后进展的在II期ATLA研究中治疗的患者中显示出实质性缓解[39]。仍需确定后线选择。迄今为止,在前瞻性临床试验中尚未发现ALK TKI治疗的理想顺序。一线艾乐替尼似乎是最佳疗法,因为它在PFS方面优于包括贯序方案的其他TKI [27]。然而,仍需等待测试新一代ALK TKI(brigatinib、lorlatinib、ensartinib)的三项一线研究的结果。需要进行临床前和临床试验以确定延迟或防止对ALK TKI治疗出现耐药性的最佳组合方案。
